Minor Supervision Acknowledgment.

The safety of minor participants is a shared responsibility between parents, legal guardians, authorized supervising adults, and independent trainers.

This Acknowledgment explains the supervision responsibilities that apply to every session involving a minor participant.

I understand and acknowledge that a parent, legal guardian, or another authorized adult designated by the parent or guardian must remain physically present for the entire duration of every training session involving a minor participant.

The supervising adult must remain:

• at the training location;
• reasonably attentive to the session;
• immediately reachable at all times; and
• capable of responding if assistance is needed.

Leaving a minor participant unattended during a session is not permitted.

I acknowledge that the supervising adult is responsible for:

• transporting the participant to the training location, unless alternative transportation has been independently arranged by the family;
• ensuring the participant arrives safely;
• remaining present throughout the session;
• ensuring the participant departs safely after the session; and
• supervising the participant immediately before and after the session.

Trainurz's responsibility does not begin before the scheduled session and does not continue after the session has concluded.

I understand and acknowledge that independent trainers are engaged solely to provide basketball instruction.

Trainers are not:

• babysitters;
• childcare providers;
• camp counselors;
• custodians;
• guardians; or
• temporary caregivers.

A trainer's responsibility is limited to providing basketball instruction during the scheduled session.

I understand that trainers participating on the Trainurz Platform are not authorized to:

• transport minor participants;
• drive participants to or from training sessions;
• accompany participants away from the agreed training location without the supervising adult; or
• provide transportation under any circumstances as part of services arranged through Trainurz.

Parents and guardians remain solely responsible for transportation arrangements.

Training sessions are expected to occur only at the location agreed upon through the booking.

If circumstances require changing the location—for example, because of weather, court availability, or safety concerns—the supervising adult must be informed before the participant relocates.

A supervising adult must remain present regardless of any approved location change.

Sessions may not be relocated to:

• private residences;
• hotel rooms;
• vehicles;
• isolated locations; or
• any setting that would reasonably compromise participant safety.

If a medical emergency or other safety concern occurs during a session, the trainer may:

• stop the session;
• notify the supervising adult;
• contact the participant's emergency contact;
• contact emergency services (911); or
• take other reasonable actions necessary to protect the participant until appropriate assistance arrives.

The supervising adult remains responsible for all decisions regarding the participant's continuing care after emergency responders or the supervising adult assumes responsibility.

I understand that if a supervising adult leaves the training location or is otherwise unavailable during a session, the trainer may:

• suspend the session;
• end the session early;
• refuse to begin the session;
• report the incident to Trainurz; or
• decline future bookings.

Any refund resulting from a failure to comply with this Acknowledgment shall be governed by the Trainurz Cancellation Policy.

I acknowledge that trainers using the Trainurz Platform are independent contractors and are not employees, agents, partners, or representatives of Trainurz.

Trainurz does not supervise training sessions or provide childcare services.

I understand that selecting an acceptance checkbox, clicking an acceptance button, submitting a booking request, or otherwise electronically accepting this Acknowledgment constitutes my legally binding electronic signature.

Trainurz may retain records of my acceptance, including:

• the accepted document version;
• acceptance timestamp;
• booking identifier;
• IP address;
• browser user agent; and
• other information reasonably necessary to demonstrate legal acceptance.

By accepting this Acknowledgment, I certify that:

• I have read this entire document.
• I understand that I am responsible for supervising the minor participant throughout every training session.
• I understand that Trainurz is not a childcare provider.
• I understand that independent trainers are engaged solely to provide basketball instruction.
• I understand that trainers are not responsible for transporting or supervising minors outside the scope of the scheduled training session.
• I agree to comply with the supervision requirements described in this Acknowledgment.
